The Expression of Wish form relates solely to any lump sum payable under the Donaldson (UK) Pension and Death Benefits Plan (defined benefit scheme), with payment subject to the final discretion of the Pension Trust Board. This form should be completed by deferred members and pensioners.
Important Notice for Active Members
The main Death in Service lump sum, previously provided by the Donaldson (UK) Pension and Death Benefits Plan, is now offered through a separate insurance vehicle or Death in Service Trust. Active members must complete a separate Expression of Wish form for this benefit, available at the HR Department (please see Members booklet, appendix point 2).
